SRS Forms and Information

SRS delivers an efficient way for forms to be updated and is “designed to reasonably lead users to compliance” for all districts using the system. KSB School Law, a leading school law firm in Nebraska, recommends SRS because it promotes better compliance than other solutions available.

SRS supports 25+ special education forms tailored to Nebraska requirements and regulations. These forms include:

  • Multi-Disciplinary Team (MDT) Report
  • Individualized Education Program (IEP)
  • Early Intervention – Individualized Family Service Plan (IFSP)
  • Progress Reports
  • Prior Written Notice
  • Other Notices and Consent forms
  • Equitable Service Plan
  • Revocation of Consent
  • Notes
  • Summary of Performance
  • Functional Assessment
  • Assistive Technology
  • Specialized Transportation

With SRS, student records are easily and securely transferred from one district to another as a student changes enrollment. Districts also may easily grant access to NDE staff as needed. It features a parent-friendly/consistent “look” from one district to another so that parents don’t have to “learn” or “navigate” another form structure.

Local support is accessible and available with a dedicated help desk staff that works solely with SRS and are the same people who do the coding and/or trainings.
